OASIS COMMUNITY CHURCH PICKLEBALL LIABILITY WAIVER AND RELEASE

ASSUMPTION OF RISK:I, the undersigned, acknowledge that participation in pickleball activities at Oasis Community Church involves certain risks, including but not limited to personal injury, falls, collisions, or other unforeseen incidents. I voluntarily assume all risks associated with my participation and accept full responsibility for any injury, loss, or damage that may occur.

WAIVER AND RELEASE OF LIABILITY:In consideration of being allowed to participate in pickleball activities at Oasis Community Church, I hereby release, waive, and discharge Oasis Community Church, its officers, directors, employees, volunteers, agents, and affiliates (collectively, “the Released Parties”) from any and all liability, claims, demands, actions, or causes of action arising out of or related to any injury, illness, death, or property damage sustained by me while participating in or observing pickleball activities, regardless of whether such injury or damage is caused by the negligence of the Released Parties.

INDEMNIFICATION:I agree to indemnify and hold harmless the Released Parties from any claims, liabilities, damages, costs, or expenses (including reasonable attorneys’ fees) that may arise as a result of my participation in pickleball activities.

MEDICAL CONSENT:I authorize Oasis Community Church and its representatives to secure emergency medical treatment on my behalf in the event of an injury or illness. I acknowledge that I am responsible for all medical costs incurred as a result of such treatment.

PHOTO/VIDEO RELEASE:I grant permission to Oasis Community Church to use photographs and/or videos taken of me during pickleball activities for promotional and informational purposes without compensation.

ACKNOWLEDGMENT OF RULES:I agree to abide by all rules and regulations set forth by Oasis Community Church regarding the use of the facilities and participation in pickleball activities. I understand that failure to comply with these rules may result in my removal from the premises.

SEVERABILITY: If any portion of this waiver is deemed invalid or unenforceable, the remaining provisions shall continue in full force and effect.
